Your Conservative Councillors in Westminster have announced plans with the NHS to provide vulnerable people with more effective joined-up care in their home, which will bring together social care, community care and mental health services through the tri-borough with The Royal Borough of Kensington and Chelsea, and Hammersmith & Fulham (H&F) Councils .
Minister of State for Health, Norman Lamb and Minister for Local Government, Brandon Lewis on June 18th praised the Tri-Borough integrated care pilot, which aims to reduce the need for hospital admissions by helping people stay independent.
Cllr Marcus Ginn, Cabinet Member for Community Care at H&F Council, said: "Hospital admissions should be seen as the last resort, not the starting point as it is all too often today. By working with GPs and the NHS to improve access to care in the home we will be helping a new generation of people live independent, healthier lives."
Norman Lamb MP, Care and Support Minister said: "By bringing together services from across Westminster, Hammersmith & Fulham and Kensington and Chelsea councils, you are leading the way when it comes to integrated health and care services. I want to take this opportunity to thank you for taking this leap forward to improve people's lives. Plans, which are set to save around £38 million net per year, will integrate health and social care support to enable more people can stay independent in their own home."
